Safety of Biologic Use During Pregnancy and Breastfeeding in Dermatology
TAKE-HOME MESSAGE
- This review summarizes data regarding the safety profile of commonly used biologics in dermatology in terms of use during the pre-conception, pregnancy, and postpartum period and provides recommendations for each period. TNF inhibitors, particularly certolizumab, show the most robust safety data for these gestational stages. There are no significant adverse outcomes reported among women using biologics during pregnancy. Owing to limited data on other classes of biologics (besides TNF inhibitors), their use during pregnancy is generally not recommended, and contraception is advised. Additionally, live vaccines should be avoided for the first 6 months in infants born to mothers exposed to biologics (except for certolizumab) in the third trimester.
- The use of biologics in pregnancy should be assessed on a case-by-case basis, with an understanding of the risks and benefits of therapy in pregnancy.

However, their use during pregnancy and breastfeeding is a subject of ongoing concern due to limited data on their safety in these populations. As the course of many inflammatory skin conditions is unpredictable during pregnancy and may worsen, biologics are important therapeutic tools for disease stabilization in this patient population. In this review, we provide a comprehensive summary of the gestational safety profile of biologics commonly used in dermatology and provide recommendations during pre-conception, pregnancy, and post-partum periods.
