    I will certainly initiate Bevacizumab. She has mild neurological symptoms that can improve immediately with this drug.
    There is some evidence that Irinotecan with Bevacizumab could be more active than with temozolomide in unmetilated patients, tha presents poor response to Temozolomide.

    Why not?, we could give temozolomide and see what happens.

    Temozolamide is an option for this patient, despite tumor MGMT promoter was not methylated, there may be benefit. It should be considered [...] irradiation after evaluation of disease progression size and time since last RT course. For this purpose, methionine PET/CT or MR spectroscopy may be helpful.
