Prophylactic Nephrectomy in Cystic RCC?

    Good question. If no kidney transplant is planned, I would think it controversial to do a contralateral prophylactic nephrectomy and general screening could be employed based on the nature of the cysts viewed. Although RCC is much more common than the general population, the rate of cystic transformation is still quite low.  If the patient is going for kidney transplant however, then it would make sense to consider prophylactic nephrectomy as this is one of the potential causes of death post-transplant, and there is a theoretical additional risk when on immune suppression. However, there are no clear recommendations and close ultrasound surveillance of the cysts would be acceptable.
