Serum electrolyte,liver function test,CT scan of chest and abdomen, Whole radionuclide bone scan or PET -CT scan,ECG,CBC.

If the mammogram and ultrasound work up doesn't yield a lesion, I would add breast MRI, because of it's increased sensitivity. PET to look for additional lesions. If no additional info is rendered, then bronchoscope/ biopsy to exclude lung tumor.

Would consider gcdfp-f15 and mammaglobulin of the lung mass, if negative do lung lobectomy . PET-CT may identify other metastatic sites

Breast cancer has history of recurrence after many years complete treatment.There should be high suspicion of secondary malignancy as the patient was exposed to cytotoxic agents.The baseline tests done were good enough to detect the primary.I was impressed with this reminder of likelihood of cancer recurring after such a long years post treatment.
