    No ALND, but Axillary  Radiotherapy , plus adjuvant chemotherapy ex: DC x4 and Hormonotherapy with A.I.

    No axillary dissection, instead radiotherapy to the axilla.
    This tumor has aggressive characteristics; ki67 40% ,3 L N  +, would give :  AC X 4  followed pacl X 12, then AI

    I would go for ALND then will give her the benifiy of oncotype Dx 
    Adjuvant radiation and hormonal therapy
