    According to ASCO Guidelines there is no a clear role for a systemic therapy in case systemic disease is not progressive at the time of brain metastasis diagnosis. However the quality of evidence is low and, sincerely, I would not feel so confident to leave such a young and high risk patient without a systemic treatment at all. Probably I would consider TDM-1, but the other problem would be....how long?
