I don't think BRCA testing could be a game-changer in this case. I prefer Oncotype as the Er is strongly positive and the patient age is seventy. I am not sure anthracycline would be the best choice.

This patient should have complete metastatic workup with a CT Chest abdomen and pelvis and bone scan or PET/CT, due to at least stage III disease and possibility of contralateral axillary involvement. If negative, could consider neoadjuvant AC->T if excellent health and echo normal. Otherwise consider neoadjuvant TC x 6. Would not do oncotype if clinically N2 disease.
