Assume her Renal function is relatively good for her age ? Since the tumor growth is slow , and patient asymptotic, favor observation over Systemic therapy which would cause side effects. Not sure abt the role of Cryiablation on the 2nd kidney. Would avoid surgery.

I would not go for genomic test... regarding the growing lesion, I would consider radiofrequency or cryoablation if feasible and the patient has an acceptable general condition. Otherwise I would observe the patient.

I would rather wait till symptomatic. Act as per the symptom when becomes dymptomatic
