Considering that the results of the CREATE-X trial are (surprisingly?) unpublished so far, I would not consider an adjuvant chemotherapy outside of a clinical trial.
Regarding the radiation therapy I would suggest post mastectomy radiotherapy with regional nodal irradiation

She needs post-op RT to chest wall and supraclaviclar fossa.
I will discuss regarding adjuvant Capecitabine.
