it's a recurrent tumor with new biology. Metaplastic ca is a bad tumor to treat, I will recommend MRM.

Geriátric assesment and therapy guided by results. Radiotherapy whole breast. Fit and life expectance more than 5years,consider patient desire and eventually Chemotherapy.

Recurrence with interim progression to endocrine insensitivity. Treat with mastectomy and surveillance then consider palliative chemotherapy if disease relapses. Not for 'adjuvant' chemotherapy now as this is a recurrence and it will not change outcome.
