    Ok We will go for MRI within 7 days .
    Sir is it 100 percent sure that recurrence will occur or there is any chance that recurrence will not occur again after GTR and radiation .

    The role of temozolomide in unmethylated GBM remains a controversial issue; however, this patient has already had 4 cycles, and I would complete an additional 2 cycles to complete 6.  Going by level 1 data, one could still potentially consider adding TTField therapy, as there is a randomized trial demonstrating survival advantage, irrespective of methylation status.  NGS has a small probability of yielding an actionable mutation, but if possible, could be considered.  Recurrence is most likely at some point down the line, although all of us have the very few exceptional patients that have become long-term survivors, and therefore, in someone who has had a gross total resection, I would never take hope away.

    Dear All 
    Thanks for the suggestions .Now the patient has completed 7 cycles of temozolomide .
    Physically and mentally fit after 11 months of surgery 
    Any other suggestions.

    11 cycles of temozolomide completed no physical imbalance in memory or in walking eating and sleeping .dr is saying to go for mri after 12 cycles .pls suggest

    I have a question for the group.  What is the optimal # of cycles for adjuvant temozolomde? Is it 6? 12?  indefinitely until progression?

    This question is infact quite interesting.There is no definite answer for this.Usually i give upto 12 cycles if the patient is tolerating well symptomatically,lab wise n imaging wise.But if you see the literature there are few anecdotal papers where TMZ was given for 101 cycles.So it is your Oncologist who has to decide

    17 months completed without any symptoms .No MRI done till now after surgery .
    Is MRI suggested ?
    No complains no symptoms .
    Walking +speech + memory all good .
