This is not an unusual story for metastatic Infiltrating lobular cancer which can masquerade and even present as linitis plastica in the stomach, infiltrating disease in the bladder often without other detectable disease. She clearly progressed on letrozole and a change from that is mandated. I would querry the choice to exemestane alone since such crossover has only minimal and largely ancetdotal evidence of benefit. One might consider adding everolimus - the Bolero. studies but this can lead to more uncomfortable side effects. In this era of cd kinase inhibitors with hormonal therapy, I would wonder about using it with fulvestrant or fulvestrant alone and favor fulvestrant with or without palbociclib for this lady.
